README, CLAUDE.md and .env.example now point at llm.toml for LLM settings
(OPENROUTER_API_KEY stays in env) and describe the legacy env override
window, the discovered picker and the startup cross-check. ADR-0003 records
the move and amends ADR-0002's deployment-config home; CONTEXT.md's
Deployment config / Hard default entries follow. Source comments and the
"set LLAMA_SWAP_URL" error strings name the llm.toml keys.
Also fixes README's AGENTIC_MAX_ITERATIONS default (10 -> 12, what the
code uses).

Records the config boundary the #127 owner-default columns establish:
.env = boot-time deployment config + secrets (never DB), user_ai_prefs
single row = per-request owner defaults (hot-tunable, no secrets),
resolution request param -> owner default -> hard default, and the
safe-by-default privacy posture for default_backend. Adds the
Configuration glossary section to CONTEXT.md (owner default,
deployment config, hard default).
- CONTEXT.md: date/time vocabulary (true instant, wall clock, camera
offset, display zone, filename date class, ...)
- ADR-0001: dates are true instants; wall clocks are a display concern
- un-ignore docs/adr/ so decisions land in git
